There have been a couple, but they're rare, and usually on a mobile lab.

Sometimes I think we're alone in the universe, and sometimes I think we're not.
In either case the idea is quite staggering.

work order notes, printed 9/25/2012, with invoice dated 2007, says ENGINE WORN OUT in 2007 with miles/hours 15454?

It did not get a NEW engine, as alleged, it got a SWING engine. A Det. SWING engine is like a Cummins RECON engine. The are REBUILT or REMANUFACTURED.
wonder how many actual total miles now.
Seeing stones behind the tires, I guess the worn out park brakes did not get replace when they were found to be bad during test drive by mechanic.
Body appears to have partial repaint (black) down both sides.

There have been a couple diesel pullers (Freightliner)that had to have a driver's door because engine house came about up to driver's shoulder -- kinda hard to step over.

Could it be so heavy it just wore out the engine?

I would guess that Barth would build anything a customer ordered. I have even seen a short Euro with a driver door.

sorry I didn't make myself clear, Kevin.
THIS Regal is not a puller. It has a nearly flat cockpit floor, beneath which I would expect to find the diesel genset.
I was thinking more along these lines:
